2. Using ALL UPPERCASE LETTERS, fill in the name(s) for each guest above.

3. For each guest, circle the appropriate MEMBER/NON-MEMBER ticket price.

Guests qualify for the MEMBER discount if they have an active membership with the Eastland Disaster Historical Society or a member of their immediate family has an active membership with the Eastland Disaster Historical Society.

4. For each guest who wishes to walk through a private showing of TITANIC: The Exhibition, circle the respective TITANIC ticket price.

Note that a private showing of the Eastland Disaster exhibit is included in the MEMBER/NON-MEMBER ($20/$25) ticket.

5. For each guest, total the respective ticket prices which have been circled and write the amount in the GUEST TOTAL box.

6. Add the GUEST TOTAL figures and write the amount in the TICKET TOTAL box.

7. If you are applying for an annual membership with the Eastland Disaster Historical Society, complete a "Membership Application" and write the amount of the annual membership fee in the "MEMBERSHIP (optional)" box.

8. Add the TICKET TOTAL and the "MEMBERSHIP (optional)" amount and write the amount in the CHECK TOTAL box.

9. Mail this completed form and payment in the amount of the CHECK TOTAL box to:
